I also did the online visa form but a few pointers - very limited characters for all address boxes so abbreviate (leave out north, south, use St for street etc), my hotel in Thamel was ward 26 and upload a Jpeg image not PDF.
I took USD to pay for our visa on arrival. I would prefer to have cash than have to rely on the ATM. The bank felt completely safe but a little more time consuming yet the transaction was professionally handled etc and I would do this again. I had to pay my trek agency about USD900 so went to Himalayan Bank in the same building as Fire and Ice Pizza in Kathmandu and got this huge wad of notes (stapled together) and a receipt (no receipt provided at the money changer in Pokhara). I usually use only the major banks in each country - in Nepal I believe they would be Himalayan, Nabil and Standard Charter. You just have to keep the account active every 6months - I really only use it for travelling. Takes about 10days to process and receive card. I do recommend it as a good card to have in your wallet. Overall it has been a good card for me as the interbank rate is better than the Combank and similar Aussie bank rates and no 3% fee (which combank charges). In all the previous transactions in Tanzania, Rwanda and Greece I have had no fees but I did get hit with a fee for the ATM in Bangkok. Hi, I attemtpted to use my card at the Himalayan Bank in Pokhara and at least it acknowledged my name after I entered my PIN etc but the transfer from Combank (Friday) didn't go thru until the Monday which was too late so no luck as I didnt have enough cash.
